SUBJECTIVE
Patient returns with continued complaints of redness, swelling, and flaking of the skin around the elbow, etiology unknown. Patient wishes to discuss the results of her biopsy.

OBJECTIVE
Erythematous rash over left posterior elbow. Multiple trials of topical steroid applications have been unsuccessful.

ASSESSMENT
Biopsy report indicates chronic psoriasis.

PLAN
Prescribed new topical corticosteroid to be applied 3–4 times daily. Prognosis is favorable if the patient is compliant.
