Instructions: Read the following discharge instructions and complete the form provided here.
Mrs. Lottie Hanvers was being treated at Davis Hospital, in room 108, by Dr. Andrews. She experienced a fall at home, sustaining a mild concussion and broken left pinkie finger. She was also diagnosed with type II diabetes during her hospital stay. Now that she is being discharged to her home, Dr. Andrews has provided the following discharge instructions for post-hospital care:
Medications
- Repaglinide 0.5 mg orally, with each meal
- Acetaminophen 500 mg x2 every six hours for pain, as needed
Diet
- Continue with diabetic diet as directed.
- Check blood sugar level with a glucose meter before every meal and keep log. If glucose level is too low, eat hard candy, or drink sugar-sweetened soda, orange juice, or a glass of milk. Then eat a small snack, such as half a sandwich, a glass of milk, or some crackers if your next meal is more than 30 minutes away. If glucose level is too high, check it again prior to your next meal. If still high, contact Dr. Andrews.
Activities
Resume normal activities of daily living, but sit down if feeling dizzy. Then check blood glucose.
Wound Care
Leave bandage on finger until follow-up appointment with Dr. Andrews. Do not get the bandage wet.
Follow-Up Care
A follow-up appointment with Dr. Andrews has been scheduled for May 19, 2017 at 11:45 a.m. Mrs. Hanvers is to schedule a follow-up appointment with Dr. Janis in three days.
