Your patient, Ken Tanaka, recently had a colonoscopy. The initial diagnosis made by the physician was colon polyps. One week later, the results of the test indicated colon cancer. Click on “Patient Activity History” to bring up the end of the previous office visit. Create an addendum to enter the new diagnosis into the patient chart. The date of the addendum should be one week after the “Date Created” and “Date of Service.” Review the guidelines in your textbook.
