Assume you are caring for the patient named Ben Jones. You work the night shift and the physician’s orders require the recording of fluid intake and output. Record the following entries for your shift. Then add up and record your shift totals and all of the 24-hour totals.

Entries: Ben had a rough night. At midnight, he vomited 100 mL and at 2:30 a.m., he vomited 150 mL. At 3:00 a.m., he vomited another 90 mL. An IV was started, and Ben received 400 mL of fluid. He was able to sip one-half juice glass of water at 5:30 a.m., and he voided 250 mL urine at 6:00 a.m.
